Keep a downloaded copy of your form just in case! I recommend at least having a scanned copy on file. Emailing your form is the best way to ensure you can forward the email again if it gets lost. CTAs, Case Managers, Councilors & other staff have access to scanners. We can scan your form for you and email it to you.

Next call (661-799-1020) or visit DOR (23838 Valencia Blvd.,Suite 303) Mon-Fri and say " I have completed my adult transition form. I would like to speak to a counselor of the day to set up a meeting". It may be helpful to have a parent or guardian present. If you do not live in SCV you may be assigned to a different DOR office. So call and check if that is the case.
